If you are interested in joining this club, please print out and fill out this PDF form and either:
- Mail it to our charter Lions Club, San Francisco Fil-Am Lions Club, PO Box 712, Daly City, CA 94017, ATTN: leo50 - Applicant's-Full-Name
- Send it as a private message to the GRYPHONS Facebook page
- Attach the application in an email to GRYPHONSLeoClub.SFFilAmLions@gmail.com with the subject "leo50 - Applicant's-Full-Name"
- Turn it in personally to one of our current members to be given to our Board of Directors at the next event.
If you decided to submit your application online, we accept scanned PDF files of the form (grayscale/black and white preferred) with the filename "leo50 - Applicant's-Full-Name.pdf".
